• The medical name of the illness that Heather Evans (Kim Basinger) was said to have in the film was "pathological intoxication" which is a true real-life medical disorder that can exist. Also known as "pathological alcohol intoxication", according to M.H. Hollender in the Journal of Clinical Psychiatry, the condition "appears as a diagnostic term in DSM-II and DSM-III, is defined in a psychiatric dictionary and is described in several textbooks".
• This was the second movie in about five years where actress Kim Basinger portrayed a character who underwent a dramatic personality change after drinking alcohol. The first film had been Blake Edwards' comedy Blind Date (1987).
• Uma Thurman's then husband, Gary Oldman, did a cameo as a mental patient client of Richard Gere's character Isaac Barr. Oldman did five days on the movie and did several scenes with Gere. Director Phil Joanou had to cut Oldman out of the movie, largely because the actor was simply too good: "He was so fantastic in the movie that when we showed the film to an audience, they wanted to see a lot more of him. It threw the whole movie out of whack. His role was just meant to be an off-beat supporting character, but audiences were pissed off that he drifted out of the story and they didn't find out what happened to him". Oldman had also appeared in Joanou's previous movie State of Grace (1990), and they also worked together in the television episode Dead-End for Delia (1993).
